 Cleanup Waiver & Release 

Hendricks County Partnership for Water Quality

In consideration of the participation of the individual named below in the specified activity/event, the undersigned parent(s) or legal guardian(s) of the participant hereby waive, release, and relinquish any and all claims, rights, and causes of action, including but not limited to those for personal injury, property damage, and wrongful death, arising from the participation of the individual in the aforementioned activity/event, wherever or however such incidents may occur and for the duration of said activity/event. By signing this Agreement, all such claims, rights, and causes of action, whether on behalf of the participant or anyone acting on their behalf, are hereby waived, released, and relinquished, and the undersigned participant or parent(s)/guardian(s) do so on behalf of themselves and the participant's heirs, executors, administrators, and assigns.

The purpose of this agreement is to exempt, waive, and release the Releasees from any and all liability to the participant or any individual or entity claiming on behalf of the participant for personal injury, property damage, wrongful death, or any other claim, right, or cause of action, regardless of whether such liability, claim, or cause of action arises from the alleged negligence of the Releasees. "Releasees" shall include the Towns of Avon, Brownsburg, Danville, Pittsboro, Plainfield, and Hendricks County, their respective employees, insurers, and any homeowner associations, sponsors, event hosts, or other participants.

The participant and/or participant's parent(s)/guardian(s) acknowledge that they understand the risks associated with the activity mentioned above/event, that they are signing this document voluntarily and with full knowledge of their actions, and that they have the legal authority to sign this Waiver and Release on behalf of the participant.

Additionally, the undersigned agrees to grant the Hendricks County Partnership for Water Quality permission to use any photographs taken during the activity in press releases or publications. The Hendricks County Partnership for Water Quality comprises the Towns of Avon, Brownsburg, Danville, Pittsboro, Plainfield, and Hendricks County.
